The Case manager is responsible for identifying, assessing, planning, coordinating, monitoring and evaluating healthcare services throughout the continuum of care for plan members in order to meet an individual?s health care needs using communication. Assumes a leadership role with all members of the healthcare team to achieve optimal clinical and resource outcomes.
BASIC HIRING REQUIREMENTS:
Associate?s Degree, Bachelor?s degree or Master?s degree program in Nursing and current, valid license to practice in North Carolina required.
Minimum of three years of current experience in an acute care hospital or outpatient based experience.
Minimum of six months case management, discharge planning, on-site concurrent review and/or home health experience required.
ADDITIONAL SKILLS:
The incumbent must possess medical management/clinical decision making skills and a sound understanding of and enthusiasm for managed care concepts/philosophy.
Excellent communication and interpersonal skills with all levels of internal and external customers necessary.
Individual must be self-motivated/self-directed.
Must possess the abilities to work independently, time management skills, and ability to prioritize effectively.
Advanced assessment and teaching skills; problem solving skills.
The ability to deal effectively with change within the unstructured nature of case management and remain focused.
Familiarity with legal terminology and liability issues and ability to handle ethical or risk management issues.
Demonstrates creativity, critical thinking, counseling, interviewing, negotiating, and research skills.
Utilization and Quality Management/Outcomes experience preferred.
Certified Case Manager (CCM) certification is preferred and required within 2 years of employment.
Knowledge of computer applications.
Previous work experience with a managed care company or provider is also preferred.
